Facility Evaluation Report

LIcensing Program Analyst (LPA) returned for an unannounced Case Management visit at the facility. LPA met with Licensee (LE) Maricel De Leon and explained the purpose of the visit.

At time of entry, three of four clients were at Adult Day Program. One client was being discharged from the hospital and was returning to the facility at 2pm. Currently the facility has a census of three clients.

LPA spoke with LE regarding the two independent clients and that they are still governed by the Title 22 Regulations. LE agreed to update the LIC 602A for both independent clients. Age exception requirements were discussed with the Licensee if 50% of the client population is over the age of 59. Licensee acknowledged this and a copy of Title 22 CCR 85068.4 - Acceptance and Retention Limitations was given.

An exit interview was conducted with Licensee Maricel De Leon and a copy of this report and regulations were given at the time of the visit.
