Licensing Program Analyst (LPA) Rose Ruppert attempted to conduct an unannounced Case Management visit at 10:40am. LPA knocked and rang the doorbell but there was no response. LPA contacted the LIcensee phone number on file and left a voice mail message. LPA did not receive a phone call back.
LPA was unable to enter the facility to conduct the visit. A copy of the report was not provided to the facility.
