Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by staff . During today’s visit, LPA met with Licensee (LE) Maricel DeLeon.
The facility is a two story home with an approved fire clearance of six ambulatory clients. The facility currently has a census of two clients in care that were both at Day Program at time of visit. Additionally there are two tenants who reside here who are background cleared and are independent. There are five bedrooms on the first floor and two bedrooms on the second floor where the Administrator and husband reside. The second floor is inaccessible to clients.
During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in two of two client bathrooms, and testing auditory devices on all exits . The hot water temperature measured between 118.3 and 119.1 degrees Fahrenheit and all smoke and carbon monoxide detectors were operational . The fire extinguisher is charged and was serviced on June 17, 2025. The facility’s last fire drill was conducted on June 1, 2025.
LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand . All appliances were in working order and the kitchen was recently remodeled. Sharps and knives are secured in the medication cabinet as the kitchen remodel was just completed. Emergency supplies were located in the garage; with additional non-perishable items.
