Licensing Program Analyst (LPA) William Vanegas made an unannounced inspection, in order to deliver findings of previous serious incident report received on February 6,2025. Upon arrival LPA Vanegas was greeted and granted entry to the facility by Direct Support Staff (DSP) Fe Pulmano, and advised DSP Pulmano of the nature of the visit.
LPA Vanegas amended previous report, left copy of the amended report, and a copy of this report at the facility, and an exit interview was conducted with DSP Fe Pulmano.
