The inspector’s account

Licensing Program Analyst, Kathrina Chin made an unannounced visit today for the purpose of conducting a Required - 1 Year evaluation. LPA Chin met with Program Director Pablo Garay, who accompanied LPA's on a tour of the inside and outside of the facility. The Day Program consists of several rooms; Activity rooms, an office, a changing room, and one client restroom with 6 toilets. LPA observed changing room to be dirty and disorganized. Poisons, toxic substances and other dangerous objects are stored and locked in a hallway closet. Basic items reviewed: furnishings, trash cans with lids, cleanliness and state of repair were inspected. Lunch is not provided since client's bring their own lunch and the program provides snacks. LPA observed two clients participating a group exercise. Smoke detectors and Carbon Monoxide detectors were inspected and were found to be operational. The last Fire Drill was conducted on 08/25/2021. Fire Extinguisher were observed to be charged. Water temperature was measured at 110 degrees F. in client restroom. Day program has no transportation car; staff use their personal car to transport clients or clients use public transportation. There are 11 staff, and 35 client via Zoom and 2 clients on the premises. Random selection of Staff and Clients files were reviewed. There is no medication handling for any of the clients. Program is providing the following activities: social skills, advocacy class, music, arts & crafts, sports, socialization and communication. No P&I money is handled at the day program.

Based on the observations made during today’s visit, there no deficiencies cited this review as per Title 22 of the California Code of Regulations.
