Complaint Investigation Report

Regarding the allegation, “Facility is not kept free of ants": The investigation revealed the following:

During the initial visit LPA Haley interviewed 6 facility staff members, including ED Tayfour. All staff members interviewed denied the allegation and could not recall seeing any ants present in the facility. Clients could not be interviewed and could not confirm or deny the complaint allegations listed above. LPA Haley toured the facility and was present for several hours and did not see any ants during the visit.

Relevant documentation was provided during the visit and ED Tayfour will be emailing LPA Haley a copy of the LIC500.

Based on the information gathered during the investigation, document review and interviews, the Department is unable to ascertain if the allegation occurred as reported. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred; therefore, this allegation is deemed Unsubstantiated.

An exit interview was conducted, and a copy of this report was provided.
