Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by staff at 8am. LPA met with the Administrative Designee (AD)/ House Manager Eduardo Arenas and explained the purpose of the visit.
The facility is a six bedroom, three bathroom single-story residence with a fire clearance for six non-ambulatory clients. Currently there are four clients in care. Two clients were dressed for the day and were awaiting transport. Another client was dressed and relaxing on the couch. A fourth client was asleep.
During the visit LPA toured the facility and inspected the carbon monoxide and smoke detectors. All detectors were operational. There are two fire extinguishers that were charged and inspected on October 8, 2025 and April 5 2026. The facility's last fire drill was conducted on March 29, 2026. The Emergency Disaster Plan was updated on August 2025.
LPA toured the kitchen and observed all kitchen appliances were in working order. The refrigerator has two-days of perishables and the facility has seven-days of non-perishable items on hand. The emergency supplies were observed in the living room storage. The First Aid Kit had all the required elements and a manual was on-site. LPA visited client bedrooms and all were clean, had linens and the required furnishings. The facility was a comfortable seventy-three degrees. LPA tested hot water temperatures in two of two client bathrooms. The hot water temperatures ranged between 113.1 to 114.9 degrees Fahrenheit.
