Facility Evaluation Report
This unannounced Case Management – Health Checks inspection is being conducted by Licensing Program Analysts (LPAs) Sean Haddad and Eboni Bentley for the purpose of a health and safety check. LPAs met with Staff #1 (S1) Alfredo Garcia and explained the purpose of the inspection. Administrator (AD) Jon Castro was not present during the inspection.
During the inspection, LPAs and S1 toured the facility. LPAs observed there were 4 staff and 5 clients present. LPAs conducted health and safety checks on the clients present and confirmed they were doing well and observed no health and safety issues. LPAs observed the facility to be clean and organized and found no health and safety issues. LPAs observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food, the electricity and water were running, the facility had soap and paper towels, and the medications, knives, and toxins were properly stored. LPAs requested and reviewed copies of the resident roster, staff roster, and staff schedule. LPAs inspected client money and ledgers for 6 clients.
There were no deficiencies observed in the areas inspected. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report was discussed with and provided to facility representative.
