Facility Evaluation Report
Case Management Deficiency Report – Amendment:
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced Case Management Deficiency visit. The purpose of this visit was to amend a previous complaint report. LPA Haddadin was greeted and granted entry into the facility by House Manager (HM) Sarah Pinero.
LPA Haddadin had previously conducted a substantiated complaint investigation on April 24th, 2025, under Complaint Control Number: 22-AS-20250411093950. This Case Management Deficiency report serves to amend the Deficiency Page from that investigation, changing the citation from its original classification as a (B) Potential health and safety violation to an (A) Immediate health and safety violation.
Due to the previous substantiated findings, the facility is being cited for a violation of Title 22, Division 6 of the California Code of Regulations.
An exit interview was conducted with the House Manager (HM). A copy of this report, along with information regarding appeal rights, was provided at the conclusion of the visit.
