The inspector’s account

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit to the facility for the purpose of completing the required one-year evaluation. Upon arrival, LPA was granted entry by Direct Support Professional (DSP) Gisela Montalvo. Administrator (AD) Elizabeth Halahan arrived shortly thereafter.

The facility is a single-story home located within a residential neighborhood. It is licensed to serve eight (8) ambulatory clients and operates at Service Level 2. LPA and AD Halahan conducted a comprehensive tour of the physical plant, which consists of five client bedrooms and three client bathrooms, in addition to a private bedroom and bathroom designated for live-in staff.

The facility was observed to be clean, sanitary, and in good repair. Client bedrooms were appropriately furnished and equipped with the required bedding, linens, and storage space. Bathrooms were clean, functional, and compliant with Title 22 regulations. Water temperatures were measured at 105.7, 114.6, and 105.0 degrees Fahrenheit, within the acceptable range. Toxins, disinfectants, sharps, and medications were observed to be securely stored and inaccessible to clients.

LPA observed an adequate supply of perishable food for two days and nonperishable food for seven days. The outdoor areas were inspected and found to be free of obstruction. The exit gate was self-closing and self-latching. The front yard contained sufficient shaded seating. The facility’s fire extinguisher was mounted, charged, and last serviced on October 1, 2025. Smoke and carbon monoxide detectors were tested and found to be operational.
