Facility Evaluation Report
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual inspection at the facility. Upon arrival, LPA was greeted and granted entry Administrator (AD), Louise Igisaiar, and the purpose of the inspection was explained. During the inspection, LPA and staff toured the interior and exterior of the facility, including the common areas, client bedrooms, kitchen, garage, and surrounding grounds. The facility is a single-story Level 4-A home consisting of six client bedrooms, one staff bedroom, three client bathrooms, a living room, dining area, kitchen, attached laundry room, two-car garage, and a swimming pool. LPA observed that the client bedrooms contained all required furnishings. The showers, faucets, and toilets were clean, sanitary, and in working order. Water temperature measured between 116.6 and 118.6 degrees Fahrenheit. LPA also observed that the facility maintained the required two-day supply of perishable food and seven-day supply of nonperishable food. In addition, the facility had an adequate emergency supply of food and water stored in the garage.Smoke detectors and carbon monoxide detectors were tested and found to be operational. The fire extinguisher was fully charged and displayed a service tag dated January 31, 2026. The swimming pool was secured by a gate that exceeded five feet in height and did not obstruct the view of the pool. The gas stove, microwave, washer, and dryer were inspected and found to be operable. Toxic chemicals, cleaning supplies, laundry detergents, and disinfectants were observed to be inaccessible to clients. The medication cabinet was locked and inaccessible to clients.Based on the observations made during today’s inspection, no deficiencies were cited pursuant to Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided to AD at the conclusion of the inspection.
