Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Administrator Stephanie Barrios at 8am and explained the purpose of the visit.
The facility is a level two, single story residential home with five bedrooms and three bathrooms with an approved fire clearance of six ambulatory clients. The facility currently has a census of three clients in care. At time of visit, one client was attending Day Program and one client was out with family.
During today's visit, LPA toured the facility and tested auditory devices; including smoke and carbon monoxide detectors. Smoke and carbon monoxide detectors were operational. LPA tested the water temperature in two of two client bathrooms. The hot water temperature measured between 109.4 to 111.5 degrees Fahrenheit. LPA observed bathrooms were clean, had closed lid trash containers and there were night lights in common hallways. LPA observed the fire extinguisher was charged and inspected on September 24, 2025. A facility fire drill was conducted on December 20 2025.
LPA inspected the facility food supply and observed more than two days perishable and seven days non-perishable food on site. A variety of fruits were available for clients to eat. Additional emergency water and supplies were located in the attached garage. Kitchen appliances and washer and dryer were operational. Cleaning chemicals were locked in a storage cabinet in the garage. Sharps and knives were secured in a drawer in the kitchen. LPA audited medications and per review, medications are being given as prescribed. A First Aid Kit had all of the required elements and a First Aid/CPR Manual is available.
