Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct a one-year Annual Inspection. LPA was greeted and granted entry by Staff #1. LPA met with Administrators Divina and Davidson Alipio and explained the purpose of the visit.
The facility is a one-story home with four client bedrooms and two staff bedrooms. The facility has an approved fire clearance for six ambulatory only. There are currently three clients in care. Upon entry at 12:30pm, two clients and three direct support staff were present. One client was attending Day Program and arrived at 1:30pm. The PUB 475 poster is posted in the main entryway.
During today’s visit, LPA toured the facility and tested the smoke alarm and carbon monoxide detectors. Both smoke and carbon monoxide detectors were operational. The hot water temperature in one of one clien t bathroom is 106.5 degrees Fahrenheit. LPA tested auditory devices on all exits . LPA noted bathroom toilets were in working order and grab bars and non-skid floormats were provided. The facility’s last fire drill was conducted on May 9, 2025 . Fire extinguishers were serviced on July 12, 2024. .
LPA toured the exterior and there were no hazards or obstruction of pathways. The swimming pool has a locked perimeter fence that is not accessible to clients. All client bedrooms had the required furnishings and clean bed linens. LPA observed two clients reclining in their rooms and one client was out for a medical appointment. LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. The First Aid Kit had all of the required elements and include a First Aid Folder.
