Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted at 8am by staff LPA met with Kim Southern & Anthony Morales, Administrators and Angelica Lara, House Manager and explained the purpose of the visit.
The facility is a single-story four bedroom, two bathroom residential home with an approved fire clearance of six ambulatory clients. The facility currently has a census of six clients in care.
During today's visit, LPA toured the facility. The facility temperature is 74 degrees Fahrenheit, was clean and there were no odors detected. The kitchen appliances were in working order, sharps and knives were secured and hazardous chemicals were stored in a locked cabinet in the garage. The facility had two days of perishable supplies and seven days of non-perishable items on hand. Additional emergency supplies were located in the garage. A weekly menu was posted on the refrigerator.
LPA inspected the smoke and carbon monoxide detectors throughout the facility. All smoke and carbon monoxide detectors were operational. The fire extinguisher was charged and was inspected on June 12, 2026. The facility's last fire drill was conducted on August 2, 2026. Two of two bathroom hot water temperatures were measured and ranged between 105.4 to 107.6 degrees Fahrenheit. All hygiene items were secured. LPA observed grab bars and non skid flooring in bathrooms. LPA toured two of four client bedrooms and two staff bedrooms. Client bedrooms had the required furnishings. The exterior of the property had no hazards or obstructions in pathways and there was ample space for clients to do activities outdoors. A shaded seating area was observed and the exterior gate was self-latching.
