Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 (S1) at 1:20pm. LPA met with Administrator (AD) John Davis and explained the purpose of the visit.
The facility is a level 3, five bedroom, three bathroom, single-story residence. One bedroom and one bathroom is solely used by live-in staff. The facility has a fire clearance for four ambulatory clients. Currently the facility has four clients in care.
During the visit, LPA toured the kitchen and observed the weekly menu posted on the refrigerator. There were two days of perishable items and seven-days of non-perishable items on-site. There were snacks and drinks available to all clients. All appliances were operational. The sharps and knives and chemicals were secured and stored underneath the kitchen sink. The washer and dryer are in a separate room off of the kitchen and all chemicals are secured. The garage had plenty of water and emergency supplies.
The home was clean and there were no odors detected. The PUB 475 poster, Theft and Loss and Resident Council policies were posted. The fireplace in the living room and had a glass covering. LPA toured the exterior of the property and observed a shaded seating area. There was ample space for activities and both sides of the home had self-latching gates and pathways were cleared of obstructions and hazards.
LPA reviewed centrally stored medications. Per review, medications are being given as prescribed. A First Aid Kit had the required elements and a First Aid Manual was on hand. A sharps and knives container was on site for blood test lancet used by one client.
