Facility Evaluation Report
On this date, Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual inspection of the facility. Upon arrival, LPA Haddadin was greeted by Licensee Imelda Samonte, who granted entry and was advised of the purpose of the visit. Together, they toured both the interior and exterior of the facility.
The facility is a one-story residence consisting of four client bedrooms, three bathrooms, a living room, kitchen, dining room, enclosed patio, and an unattached two-car garage. The licensed capacity is four clients, and the census at the time of inspection was four clients. During the visit, clients were observed preparing for their day program. All client bedrooms were inspected and found to be appropriately furnished with a bed, chair, and clean linens, provided adequate storage, and free of tripping hazards. The kitchen was in good repair, with all major appliances operational. Cleaning supplies, sharps, and knives were stored securely and were inaccessible to clients in care. The exterior of the facility was well maintained. Furnishings were in good repair, pathways were free of hazards, and the grounds appeared safe and inviting, with a shaded seating area available.
Fire extinguishers were observed to be fully charged, with the indicator in the green zone and the last inspection date noted as January 13, 2025. Hot water temperatures in all three bathrooms were measured between 113.4°F and 116.4°F. The last emergency drill was conducted on August 1, 2025.
