Facility Evaluation Report

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced case management visit to the facility. Upon arrival, LPA Haddadin was granted entry and met with Administrator (AD) Susan Lee, to whom the purpose of the visit was explained.

On March 27, 2026, the office received an incident report from the facility stating that Resident 1 (R1) had been transported to the hospital following an unwitnessed fall. The report did not include any further information regarding R1’s condition or status. During the visit, LPA reviewed the facility’s records and the hospital discharge paperwork. Based on the information obtained, no deficiencies were cited. An exit interview was conducted, and a copy of this report was provided to the facility.
