Facility Evaluation Report

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced case management visit. LPA Haddadin was granted entry into the facility and met with Administrator (AD) Susan Lee, to whom the purpose of the visit was explained.

On December 17, 2025, the office received an incident report from the facility stating that Resident (R1) was transported to the hospital due to a spinal fracture related to a previous doctor’s appointment, and that R1 was discharged the same day.

During the visit, LPA reviewed the facility’s records and the physician’s report. The physician’s report indicated that R1 had previously attended a doctor’s appointment during which R1 experienced a fall, and the same physician subsequently determined that the injury was a spinal fracture.

No deficiencies were cited. An exit interview was conducted, and a copy of this report was provided to the facility .
