Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Licensee Noel Villegas and explained the purpose of the visit. The facility currently has four clients in care of which three are out at day program.
The facility is a one-story home with three shared client bedrooms, two client bathrooms, one staff room, kitchen, dining room, living room and backyard. Facility appears clean, safe and sanitary. LPA observed all client bedrooms had the required components and furnishings. LPA observed the staff bedroom to be locked, making it inaccessible to clients in care. LPA observed all client restrooms to have toilet paper, paper towels and non-slip mats. LPA tested the water to be between 112.6-114.2 degrees Fahrenheit. LPA observed extra clean linens in the hall cabinet located next to the kitchen. LPA observed the kitchen to be clean and free of vermin. LPA observed the sharps to be in a locked drawer located next to the dishwasher, making them inaccessible to clients in care. LPA observed a two-day perishable and seven-day non-perishable food supply on hand. LPA observed the toxins and chemicals to be locked in a cabinet located in the laundry room next to the kitchen and made inaccessible to clients in care. LPA observed a fire extinguisher charged and with a service date of July 10, 2024. LPA observed the centrally stored medication to be in the locked closet by the front door making them inaccessible to residents in care. LPA observed the emergency food and water supply located in the dining room. LPA observed the backyard to be free of obstructions. LPA observed a shaded seating area for client use. LPA and staff tested the fire alarm and smoke detector, and they were found to be operational.
