Facility Evaluation Report
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual inspection at the facility. Upon arrival, LPA Haddadin was greeted by Cynthia Castro, Care Staff, who granted access to the facility. The purpose of the visit was explained, and Administrator (AD) Arnold Andel arrived shortly after . During the inspection, LPA and the licensee toured the interior and exterior of the facility, including the common areas, client bedrooms, kitchen, garage, and backyard. The facility is a one-story home consisting of six client bedrooms, one staff bedroom, two bathrooms, and an attached two-car garage. All client bedrooms were observed to have the required furnishings, and each bed was equipped with linens and blankets. All windows were screened, and the backyard contained a shaded sitting area for client use. The bathrooms were observed to be clean and free of debris and mildew. Faucets and toilets were operational, and the water temperature measured between 108.6 and 109.6 degrees Fahrenheit. LPA observed that the facility maintained a two-day supply of perishable food and a seven-day supply of nonperishable food, as required by regulations. Smoke detectors and carbon monoxide detectors were tested and found to be operational. The fire extinguisher was fully charged and had a service tag dated July 21, 2025. The gas stove, microwave, washer, and dryer were inspected and found to be in working order. Toxic chemicals, cleaning supplies, laundry detergents, and disinfectants were observed to be inaccessible to clients. The medication cabinet was locked and also inaccessible to clients. Based on the observations made during today’s inspection, no deficiencies were cited pursuant to Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided to AD at the conclusion of the inspection.
