Complaint Investigation Report
During the course of the interviews with staff, Staff 1 (S1) reported that the facility has enough staff to care for the clients. S1 reported that two staff members were caring and supervising Client 1 (C1) during the day of the incident. Per S2, four staff were working to care for three clients on April 25, 2025. S2 reported that if staff prevent C1 from doing what she wants that C1's behavior will only escalate. S2 stated that C1 can be unpredictable and reported that C1 can be uncontrollable. During the investigation LPA reviewed documents including the Individual Program Plan (IPP) dated July 30, 2024, for C1. Per IPP, C1 will refuse to comply with staff requests to carry out activities of daily living or stop inappropriate behaviors.
Based on the information gathered during the investigation and review of documents obtained, LPA is unable to ascertain if the allegation occurred as reported due to conflicting information. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove or refute the alleged violations occurred; therefore, this allegation is deemed UNSUBSTANTIATED.
For today’s visit, there were no citations issued per Title 22, Division 6 of the California Code of Regulations.
LPA conducted an exit interview with Licensee Noel Villegas, and a copy of this report was provided to the facility.
