Facility Evaluation Report
On today's date Licensing Program Analyst (LPA) William Vanegas conducted an unannounced visit for the purposes of completing an annual inspection. Upon arrival LPA was greeted and granted entry to the facility by facility staff. LPA explained the purposes for the visit and Administrator (AD) Noel Villegas was notified via telephone, and arrived shortly after to assist with the annual inspection. (LPA) observed the following AD has a valid administrator certificate valid from June 14, 2024 through June 13, 2026.
The facility is a one storied home with eight bedrooms two of which are staff rooms, three bathrooms whih are all shared, and an attached two car garage. LPA observed kitchen area to be clean and free of any mildew and debris, LPA observed a gas stove, microwave, and a refrigerator. All appeared to be in good repair and tested operational. LPA observed there to be a two day supply of perishable food and a seven day supply of non-perisable food on hand. A sufficient amount of emergency water was also observed to be on hand.
LPA observed all client rooms to be clean and free of any hazards and debris. LPA observed all client rooms to be large enough to walk about freely, and for them to have all required furnishings such as a chair, reading lamp, a bed, clean linens in good repair; meaning no strains or tares, a chest of drawers, and enough storage space to store away personal belongings.
LPA observed all restrooms to be clean and free of any mildew and debris. LPA observed all water faucets and toilets to be operational. LPA observed restrooms to have all required furnishings such as grab bars, and slip resistant floor mats. Hot Water temperature tested between 108.9 and 112.1 degrees Fahrenheit.
