Complaint Investigation Report
CONTINUED FROM FORM LIC9099 - LPA accompanied with administrator toured unit 1 along with the adjoining shared bathroom. The current census was requested and obtained, along with a copy of the in-service training and procedure for the prevention and treatment of bed bugs infestations.
Regarding the allegation that Staff does not keep the facility free of pests, the following has been concluded: Based on observations made during two facility visits, along with interviews with staff and clients as well as records reviewed, it was established that bed bugs had occasionally been found to be present in some of the units in the past. However, no evidence or interviews indicated an ongoing or a recently occurring infestation. In addition, facility staff provided LPA with a current contract with a pest control provider valid at the time of the initial visit, along with evidence of staff training directed at preventing and treating potential infestations, with an increased vigilance during the hotter months. The allegation is therefore found to be Unsubstantiated, meaning that although the allegations may have happened or is valid, there is not a preponderance of evidence to demonstrate that the alleged violation occurred.
Regarding the allegation that Clients in care are not provided proper food service, the following has been concluded: Based on observations made during two facility visits, along with interviews with staff and clients as well as records reviewed, it was confirmed that facility kitchen staff are following the posted menus for the planning of meals served. Menus reviewed included the components of a balanced diet of a nature to meet the dietary needs of individuals in care. Outside of posted meal times, additional servings can be provided to clients absent during the main meal service along with snacks being provided on demand and/or during the bedtime medication rounds. As a result, the allegation is found to be Unsubstantiated, meaning that although the allegations may have happened or is valid, there is not a preponderance of evidence to demonstrate that the alleged violation occurred.
Regarding the allegation that Facility does not have heaters, the following has been concluded: A majority of units are observed to be equipped with built-in wall heating units, however other units are not. An ample supply of screened heating units was demonstrated to be present and available upon demand or when the need is present. Therefore, the allegation is found to be Unsubstantiated, meaning that although the allegations may have happened or is valid, there is not a preponderance of evidence to demonstrate that the alleged violation occurred.
An exit interview was conducted and a copy of this report was provided to a facility representative.
