Facility Evaluation Report
On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility in order to follow up on the plan of corrections issued after a case management visit led on May 22, 2023 during which deficiencies had been observed and cited.
LPA accompanied with administrator toured unit 1 along with the adjoining shared bathroom. The physical plant is observed to be in clean and sanitary conditions, as prescribed by the Plan of corrections. A client interview confirms that staff has been keeping the bathroom clean. The administrator added that additional staff had been hired for that purpose and that extensive in-service training and documentation of cleanliness checks had been implemented.
As a result, LPA was able to clear the citation issued on May 22, 2023. An exit interview was provided and a copy of this report along with a clearance letter were provided during the visit.
