The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a case management Deficiencies visit. LPA was greeted and granted entry by staff. LPA met with Direct Support Professional (DSP) Darrell Tormis. DSP called Administrator (AD) Mitra Dixon and discussed the purpose of the visit. AD informed LPA that they are out of town and DSP can assist with the visit.

During the investigation of complaint control #22-AS-20250205110830, LPA was unable to review Client #1 (C1)s facility file due to the facility no longer having it and a deficiency was noted per Title 22 Division 6 of the California Code of Regulations.

AD informed LPA that C1 was at the facility from January 11, 2025, until February 2, 2025.

An exit interview was conducted and a copy of this report along with LIC809D and appeal rights were left at the facility.
