Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a plan of correction (POC) visit for deficiencies that were issued during the required annual inspection on April 9, 2026. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Clifford Valmeo and discussed the purpose of the visit.

Regarding the Type B citation 80066(a)(10) . LPA reviewed 2 of 2 staff health screenings dated April 15, 2026, with records of a TB test. Therefore, the citation has been fulfilled.

Regarding the Type B citation 80075(f) . LPA reviewed 2 of 2 CPR certifications completed on April 21, 2026. Therefore, the citation has been fulfilled.

LPA toured the facility and observed that the back fence has been repaired and that the walls and windowsills have been cleaned.

Based on today's observations no citations are being noted per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report was left at the facility along with the POC clearance letter.
