Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit for the purpose of conducting a Plan of Correction inspection for a deficiency issued on March 26 th , 2025 during the required annual inspection. LPA was greeted and granted entry by staff. House Manager (HM) Clifford Valmeo called Administrator (AD) Brandon Penalosa and explained the reasoning for the visit.

LPA toured the facility to do a health and wellness check and no clients were present at the time of inspection. During the inspection on March 26th, 2025 LPA gave a citation for CCR 80068(a) which stated the licensee shall complete an individual written admission agreement with each client. LPA requested the documents needed to clear the citation and HM was able to provide the proof of individual written admission agreements obtained with a signature.

Based on today’s observation the Plan of Correction has been fulfilled, thus clearing the Type B deficiency for CCR 80068(a).

An interview was conducted with House Manager Clifford Valmeo. A copy of this report and the letter of cleared deficiency was provided.
