The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Direct Support Staff (DSP) Perlita Cadiz and discussed the purpose of the visit. DSP contacted Licensee Imelda Cadiz and was able to assist by phone.

The facility is a one story home with three client bedrooms, one staff room, one client bathroom, one staff bathroom, living room, kitchen and attached two car garage. The facility appears clean, safe and sanitary. The facility currently has two clients in care. LPA observed required departmental postings on the living room wall. LPA observed the centrally stored medication to be in a locked cabinet located in the living room and made inaccessible to clients. LPA observed the emergency food supply to be in the hall closet by the living room. All client bedrooms had the required components and furnishings. LPA observed the kitchen to be clean and free of vermin. LPA observed a two day perishable and seven day nonperishable food supply on hand. LPA observed the toxins and chemicals to be in a locked cabinet under the kitchen sink. LPA observed the sharps to be in a locked cabinet under the microwave. LPA observed a fire extinguisher in the kitchen charged and with a service date of June 6, 2025. LPA observed the client restroom to have toilet paper, paper towels and a nonslip mat in the shower. LPA tested the water to be at 116.2 degrees Fahrenheit. LPA observed the staff room to be locked and made inaccessible to clients in care. LPA observed the garage to be locked and made inaccessible to clients in care. LPA observed the emergency water supply to be stored in the garage. LPA observed the backyard to have a shaded seating area for client use. LPA observed the backyard to be free of debris and obstructions. LPA and DSP tested the fire alarms and smoke detectors and they were found to be operational.
