Facility Evaluation Report
Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to conduct an Annual Required Evaluation. LPA was greeted and granted entry at 1pm by the Administrator (AD) Cora Kon and LPA explained the purpose of the visit.
The facility is a single story, five bedroom, two bathroom residential home with an approved fire clearance of six non-ambulatory clients. Currently the home has four clients. At time of visit, all four clients were attending Adult Day Programs (ADPs) but returned home after 3pm. LPA spoke with licensee regarding fees that were due. Licensee paid fees during the visit.
During today's visit, LPA toured the facility and observed a large living room that is adjacent to a dining room area. Medications were secured in a locked cabinet. The home was renovated in the past three years. All appliances were in working order in the kitchen. Sharps and knives, as well as cleaning chemicals, were secured in a cabinet and underneath the sink. The facility retained two days of perishable food items and seven days of non-perishable food items. Emergency supplies and water were observed in the garage with a washer and dryer.
Client rooms were observed to have clean linens and the required furnishings and the bathroom hot water temperature was measured at 107.1 degrees Fahrenheit. The client bathroom's lights were in working order. Smoke detectors were inspected and tested and were operational. The fire extinguisher was charged and inspected on February 11, 2026. The facility's last fire drill was conducted on March 21, 2026 and a fire drill will be conducted on this date for the second quarter.
