The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct a plan of correction visit to clear citations that were given during the annual inspection on January 20, 2026. LPA arrived at the facility at 10:30AM and did not get an answer at the front door. LPA called Administrator (AD) Cora Kon and informed her of LPAs arrival. Cora informed LPA that staff left the facility due to clients being out in the community and arrived at 10:50AM to assist LPA.

LPA reviewed current liability insurance with an expiration date of January 15, 2027, for the facility, thus clearing citation 80022(a) .

LPA observed the staff medications to be inaccessible to clients in care and locked in the staff room closet, thus clearing citation 80087(g ).

LPA reviewed a current surety bond with an expiration date of January 14, 2028, thus clearing citation 80025(b).

LPA reviewed a current medical assessment obtained by AD for client #1 (C1), thus clearing citation 80069(b).

LPA reviewed a current disaster plan for the facility, thus clearing citation 80023(a).

LPA printed citation clearance letters and gave to AD at the time of the visit.

An exit interview was conducted and a copy of this report was left at the facility.
