The inspector’s account

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual visit. LPA was granted entry and met Licensee Eunice Reyes and the purpose of the visit was explained. The home consists of 5 resident bedrooms, living room, dining room, and kitchen as well as 3 restrooms. LPA toured the facility and inspected all prospective residents’ bedrooms. Each contained the required furnishings and elements. All three restrooms were inspected and found to be clean, functional, and equipped with all required supplies. No mold or mildew was observed. Hot water was tested and measured at 105 degrees Fahrenheit. At the time of the visit, the facility had five residents in care. Knives, sharps, and cleaning chemicals were stored securely and were inaccessible to clients in care. Fire extinguishers were observed on site, with last inspection date of August 22, 2025. LPA checked two clients’ files and two staff files in which all had the required documents. Last emergency drill was conducted on December 21, 2025. The facility maintained two-days perishable and seven days nonperishable food supplies per regulations. The outdoor area included shaded seating with one table and chairs available for residents use. Emergency exits were observed to be clear and free from obstruction.

No deficiencies were observed during the inspection. An exit interview was conducted, and a copy of this report was provided to Licensee at the conclusion of the visit.
