The inspector’s account

Licensing Program Analyst (LPA) Claudia Gutierrez made unannounced visit for the purpose of conducting a closure inspection. LPA was greeted and granted entry by Licensee Mariela Oblepias and the purpose of the inspection was discussed.

On July 7, 2025, Licensee contacted LPA via email regarding their intent of closing the facility. On October 1, 2025, Licensee notified LPA via email all five facility clients had been relocated effective September 28, 2025, and they had ceased all operations. LPA informed Licensee that a closure visit would be conducted to ensure no clients are in care and Licensee stated they understood.

During today’s visit, LPA and Licensee conducted a tour of the facility. LPA observed the facility to be free of all furnishings and there were no clients in care. Licensee stated they have sold the property and will no longer have control after today’s date. Licensee provided LPA with a list of clients’ relocation addresses and phone numbers and surrendered their license.

Based on observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided at the end of the inspection.
