Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct the required unannounced annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Lynette Dizon. This facility currently has six clients of which five are at day program.
The facility is a one story home with four client bedrooms, two bathrooms, kitchen, dining room, living room, staff room, staff bathroom, backyard and detached two car garage. Facility appears clean, safe and sanitary. LPA observed the required department postings throughout the facility. LPA observed that all client rooms had the required components and furnishings. LPA observed that the staff room is locked and inaccessible to clients in care. LPA observed that the facility has extra linens stored in a cabinet in the hallway closet. LPA observed that the restrooms are stocked with soap, toilet paper and paper towels. Hot water measured at 117.8 degrees Fahrenheit and 120.0 degrees Fahrenheit in the client bathrooms located in the hallway and by the kitchen. LPA observed the fire extinguisher located in the kitchen to have a service date of February 21, 2025. LPA observed the knives and sharps to be locked in a drawer located by the kitchen sink inaccessible to clients in care. LPA observed a two day perishable and seven day non-perishable supply of food on hand. LPA observed medication is kept locked in a cabinet in the kitchen and inaccessible to clients in care. LPA observed toxins locked under the kitchen sink and in the locked garage making them inaccessible to clients in care. LPA observed facility has emergency food and water supply stored in the garage. LPA observed that the garage has an extra refrigerator and freezer for food storage. LPA observed the emergency food and water supply stored in the garage. The backyard has a shaded seating area for client use. LPA observed a locked shed in the backyard that is used for storage and made inaccessible to clients in care.
