The inspector’s account

On today's date Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of conducting an annual inspection. LPA Vanegas was greeted and granted entry to the facility by care staff. LPA explained the purpose of the visit, and Administrator (AD) (Lynette Dizon) was notified via telephone and arrived shortly after to assist with the annual inspection. LPA Vanegas accompanied with AD began a tour of the facility and observed the following. AD has a valid administrator certificate that is valid from January 21, 2026 through January 22, 2028.

This is a one storied home with seven bedrooms four of which are client rooms and three of which are staff rooms. There are two bathrooms which are used for staff and clients and an attached two car garage. LPA Vanegas observed kitchen area to be clean and free of any mildew and debris. LPA Vanegas observed a gas stove, dishwasher, micro wave, refrigerator, washer and dryer. All appliances were observed to be operational. LPA Vanegas observed all sharps and toxins to be locked away and inaccessible to clients in care. LPA Vanegas observed a two day supply of perishable food and a seven day supply of non-perishable food on hand.

LPA Vanegas observed all client rooms to be clean and free of any obstructions blocking entry ways. LPA observed all rooms to have required furnishings such as a chair, bed, chest drawers, reading lamp, enough storage space to store personal belongings, and clean linens in good repair; meaning no strains or tares.

LPA Vanegas observed all restrooms to be clean and free of any mildew or debris. Toilets and water faucets tested operational, restrooms had all required furnishings such as grab bars, slip resistant floor matts, and a shower chair. Hot water temperature tested between 116.3 and 120.4 degrees Fahrenheit.
