Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough arrived at the facility to conduct a case management follow up visit on an incident report that was submitted to the Orange County Regional Office on May 20, 2025. LPA was greeted and granted entry by staff. Staff called Administrator(AD) Martin Lopez and LPA explained the purpose of the visit. AD gave Caregiver Alex Navarro permission to assist LPA with the inspection.

The incident report submitted to the Orange County Regional Office stated that on May 19, 2025 Client#1 (C1) had acts of aggression towards staff and other clients and that the C1 had broken a window. The incident report stated that updates on a 30 day eviction notice will be given to agencies. LPA received an eviction notice regarding C1 on May 27, 2025 with an eviction dated May 20, 2025. A technical violation was given at the time of the visit. LPA toured the facility and did a health and safety check on clients in care. LPA observed all broken windows have now been fixed.

Based on today’s inspection no deficiencies are being noted per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report along with technical violation was given at the time of inspection.
