The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Mindy Fajardo and discussed the purpose of the visit. The facility currently has three clients in care.

The facility is a one story home with four client bedrooms, staff bedroom, three bathrooms, kitchen, living room, dining room, staff office, laundry room and detached two car garage. LPA observed the client bedrooms to have all the required components and furnishings. LPA observed the staff bedroom to be unlocked with medications on the dresser making them accessible to clients in care. LPA observed facility staff lock the bedroom door making them inaccessible. LPA observed the facility to have an extra clean supply of linens for client use in the hall closet. LPA observed the client restrooms to be stocked with toilet paper, paper towels and textured shower flooring. LPA tested the water to be between 117.5 and 119.1 degrees Fahrenheit. LPA observed fire extinguishers by the client bathroom and in the kitchen to be charged and with a service date of October 14, 2025. LPA observed the kitchen to have a bug trap located in the cupboard marked clients at the entrance of the kitchen with a bug in it. LPA observed the facility to have a two day perishable and seven day nonperishable food supply on hand. LPA observed extra food storage in the hall closet, storage room and garage. LPA observed the appliances to be operational. LPA observed the emergency food supply to be in the staff office. LPA observed the centrally stored medication to be in a locked cabinet in the dining room. LPA observed the garage to be used as an extra storage space for the facility. LPA observed the backyard to be free of debris and obstructions. LPA observed a shaded outdoor seating area for client use. LPA and AD tested the carbon monoxide detectors and smoke alarms and they were found to be operational.
