The inspector’s account

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced visit for the required one-year inspection. LPA explained the purpose of the visit and was greeted and granted entry by Administrator (AD) George Lozano.

At the time of the visit, the facility reported three clients in care; however, LPA did not observe any clients on the premises as all clients were attending a day program.

The facility is a one-story home with four bedrooms, two bathrooms, a kitchen, dining room, living room, backyard, and an attached one-car garage.

LPA and AD tested the smoke detectors and carbon monoxide detectors in the common areas and resident bedrooms, and all devices were operational. Resident bedrooms were observed to have the required furnishings, including appropriate bedding, linens, and adequate closet and drawer space to accommodate each client comfortably. LPA observed fire extinguishers mounted in the kitchen and garage areas with a last inspection date of January 16, 2026. Upon review of records, the emergency drill log indicated the last drill was conducted on January 3, 2025. LPA observed that sharps and knives were secured in a locked area in the kitchen and were inaccessible to clients in care.

Restrooms, toilets, and water faucets were observed to be operational. Grab bars were secure, showers were observed to be free of mold and mildew, and water temperature measured between 113.9 and 114.6 degrees Fahrenheit. {***CONTINUE-9099C***}
