Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility for the purpose of a case management. LPA was greeted and granted entry by staff. LPA met with Administrator (AD) Michelle Domingo and discussed the purpose of the visit.

The Orange County Regional Office received an email on Monday October 20, 2025 where the Administrator informed Client #1s (C1) conservator that they will be discharged on October 18, 2025. The regional office did not receive a formal eviction notice for C1. AD informed LPA that the conservator was in agreement with the resident leaving the facility on October 18, 2025, and provided LPA with a copy of the correspondence. LPA discussed Eviction Procedures with AD and provided the regulations at the time of the visit.

AD agreed to submit a new policy on eviction procedures and to update them from using the term "Discharge" and will email to LPA by November 7, 2025.

A technical violation was given at the time of the visit and a copy of this report was left at the facility.
