Facility Evaluation Report

An Informal Conference was held on this date at the Orange County Adult and Senior Care Regional Office in Orange, California. The informal meeting process was explained to the Licensee.

At this informal conference, present were: Licensing Program Manager (LPM) Kevin Saborit-Guasch, Licensing Program Analyst (LPA) Hanna Gough and Licensee Carol Wilson. The purpose of the meeting was to discuss the change of ownership of the facility.

The following concerns were discussed during the meeting:

§ Provide residents with 60 days’ notice regulations

§ Provide resident/responsible party that facility is undergoing a change of ownership

§ Provide resident/responsible party with letter of closure

§ Provide resident/responsible party that they have the right to stay or relocate to another licensed facility.

An exit interview was conducted and a copy of this report was provided to Licensee.
