Facility Evaluation Report
Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual required visit. Upon arrival, LPA Haddadin was greeted by Administrator Mario Antonio, and the purpose and scope of the visit were explained. The facility is a one-story home consisting of three client rooms, one client bathroom, one staff room, one staff bathroom, an office, kitchen, dining room, living room, laundry area, a two-car garage, and a backyard with a covered patio. During the inspection, LPA Haddadin toured the physical plant to assess compliance with applicable health and safety standards. The inspection included testing smoke detectors, verifying hot water temperatures in the facility restrooms, and testing the auditory devices at the exits. Client bedrooms were observed to contain the required furnishings and bed linens, as well as adequate closet and drawer space to accommodate residents comfortably. An evacuation chair was observed and maintained on site for emergency use. All smoke detectors were operational. The fire extinguisher was charged, with the most recent inspection documented on December 2, 2025. The facility maintained a minimum of seven days of non-perishable food and two days of perishable food on hand. Knives and cleaning supplies were secured in a separate kitchen cabinet. LPA Haddadin also inspected medication storage and the first aid kit, and observed the kit contained the required components and that medications were stored in a locked area. LPA Haddadin reviewed two staff files and two client files and confirmed required documentation was current at the time of the inspection. The most recent emergency disaster drill was conducted on January 2, 2026. Based on observations and record reviews completed during today’s inspection, no deficiencies were cited pursuant to Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and a copy of this report was provided to Administrator Mario Antonio.
