The inspector’s account

Licensing Program Analyst (LPA) Samer Haddadin attempted to conduct an unannounced annual inspection visit at the facility on this date. Upon arrival at 08:02 AM, LPA Haddadin signaled his presence by ringing the doorbell and knocking on the entrance door. After receiving no response, it was determined that no personnel or clients were present at the facility.

LPA Haddadin subsequently contacted the facility via telephone. The Authorized Representative (AD) answered the call, at which point the LPA identified himself and stated the purpose of the visit was to conduct the mandated annual inspection. The AD informed the LPA that he was currently at a dental appointment and that the two clients residing at the facility had accompanied him to the office. The AD clarified that the medical appointment was for his own personal care rather than for the clients.

In response to this information, LPA Haddadin advised the AD that the inspection could not proceed at this time and that he would return on a future date to complete the evaluation. The attempted visit concluded at approximately 08:26 AM.
