The inspector’s account

Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit to the facility for the purpose of delivering an amended version of deficiency page (LIC809-D) cited during complaint investigation and generated on November 24, 2025. LPA was greeted and granted entry by Staff Cheryl Espiritu. Assistant Administrator (AAD) Anamarie Santos was contacted by phone and arrived at 3:20 p.m.

LPA discussed the amendment with AAD. An exit interview was conducted and a copy of this report was provided at the end of the visit.
