Facility Evaluation Report
Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit to issue a deficiency in connection to a complaint investigation control number: 22-AS-20231122145751. LPA met with Assistant Administrator (AA) Anamarie Santos and explained the reason for the visit.
The investigation revealed that the facility did not maintain records of the orientation for Staff #1 (S1) and Staff #2 (S2). The on-site orientation according to the Staff Training Plan covers the Program Design, Client's Individual Program Plan, Client Rights, Medications, Health and Emergency Procedures, Special Incident Reports, and Client Abuse Reporting. Although S1 and S2 confirmed receiving training, facility failed to maintain the records for LPA's review.
Therefore, a deficiency is being cited on the attached LIC809-D as per the Title 22, Division 6, Chapter 1 of the California Code of Regulations.
An exit interview was conducted with Assistant Administrator Anamarie Santos, and a copy of this report including the LIC-811 and the appeal rights were provided at exit.
