Facility Evaluation Report
Licensing Program Analyst (LPA) Jessica Cho made an unannounced visit for the purpose of conducting a Case Management visit due to observed deficiencies in connection to Complaint Investigation Control Number: 22-AS-20231027154232. LPA met with Administrator (Admin) Edgardo Pascual and explained the reason for the visit.
It was determined during the course of the investigation that Client #1 (C1) had been issued a 30-day eviction notice on November 15, 2023. Admin confirmed that the Department was not made aware of such eviction notice. LPA also verified that the Department had no records for the eviction. LPA also conducted a tour of the facility on today's date and observed one small cockroach in the kitchen and flies throughout the facility which was also corroborated by two out of four client interviews. Admin stated that the cockroach has been treated in house. However, based on the investigation, the cockroach issue has existed since approximately October 2023, and therefore, the treatment has not been effective since.
Based on the observation made, interviews which were conducted, and the record that was reviewed, deficiencies are being cited on the attached LIC809-D.
An exit interview was conducted with Administrator Edgardo Pascual, and a copy of this report including the LIC809-D and the appeal rights were provided at the end of the visit.
