Licensing Program Analyst (LPA) Dwayne Mason Jr. arrived at the facility at 11:40am to conduct a required annual inspection. LPA was not greeted after ringing the doorbell and knocking. LPA called the facility's phone but no one answered. LPA called the facility again at 11:55am. Administrator (AD) Edguardo Pascual answered the phone. AD stated that most clients were at day program and that he had to take one client to an appointment. AD stated they would return to the facility before 1:00pm.
