On 07/11/2025 Licensing Program Analyst (LPA) Cheyenne Ratajczak arrived at the facility unannounced to conduct a case management visit. LPA met with Program Director Sona Hakobyan and explained the purpose of the visit. The purpose of the case management visit is to follow up on a meeting the facility had with the Regional Office on 05/30/2025.
During today’s visit, the LPA is requesting the following documents:
Program Director stated facility is still working on obtaining the building permit.
LPA obtained the documents during today’s visit.
Exit interview and a copy of the report was left at the facility.
