Facility Evaluation Report
On today's date March 05, 2026 Licensing Program Analyst (LPA) William Vanegas made an unannounced visit for the purposes of conducting an annual inspection. LPA Vanegas was greeted and granted entry to the facility by facility staff. Administrator (AD) Paul Chea was notified via telephone, and arrived shortly after to assist with the annual inspection. Accompanied with the AD LPA conducted a tour of the facility and observed the following. AD has an active administrative certificate valid from July 22, 2025 through July 21, 2027.
The facility is a one storied home with four bedrooms and two bathrooms, all bedrooms are used for clients and restrooms are shared. There is an attached two car garage. LPA observed kitchen area to be clean and free of any mildew and debris. LPA observed a gas stove, microwave, refrigerator, washer, and dryer. All tested operational and in good repair. LPA observed a two day supply of perishable food, and a seven day supply of non-perishable food on hand. LPA observed sharps and toxins to be locked away and inaccessible to clients in care.
LPA Vanegas observed all client rooms to be clean and free of any mildew and debris, and free of any obstructions blocking the entry ways. LPA observed all client rooms to have all required furnishings such as a bed, chest drawer, a chair, a reading lamp, and clean linens in good repair; meaning no strains or tares.
LPA observed all restrooms to be clean and free of any mildew and debris, water faucets and toilets tested to be operational. Bathrooms consisted of required furnishings such as grab bars, slip resistant floor matts, and a shower chair hot water tested between and degrees Fahrenheit.
