The inspector’s account

Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual inspection of the facility on May 8, 2026. Upon arrival, LPA Haddadin was greeted by Administrator Brian Estorba, who granted entry. LPA explained the purpose of the visit, and Administrator Estorba accompanied LPA during a comprehensive tour of the interior and exterior areas of the facility. The facility is a one-story residence consisting of a two-car garage, five bedrooms, including one bedroom designated for staff, three bathrooms, a dining area, living area, and kitchen. At the time of the inspection, five residents were in care. Resident bedrooms were observed to be in good condition and were furnished with beds, chairs, clean linens, and adequate storage space. Walkways throughout the facility were observed to be clear and free of tripping hazards. LPA tested the facility’s safety equipment, including manual smoke detectors, carbon monoxide detectors, and exit alarms. All safety equipment tested was observed to be operational. The bathrooms were observed to be clean and equipped with the required grab bars. Cleaning supplies and hazardous chemicals were observed to be securely stored in the garage and inaccessible to residents in care. The kitchen was observed to be clean and well maintained. The hot water temperature was measured at 115.3 degrees Fahrenheit, which is within the required range. The medication cabinet located in the kitchen was observed to be locked and secure. During the administrative review, LPA reviewed three resident files and compared the residents’ medications. No discrepancies were observed. LPA also reviewed two staff files, which contained the required documentation. Facility records indicated that the most recent quarterly emergency disaster drill was conducted on February 14, 2026. {***CONTINUE809C***}
