Facility Evaluation Report

Licensing Program Analyst (LPA) William Vanegas made an unannounced inspection for the purposes of delivering findings in regards to a special incident report received on February 6, 2025 and to deliver an amended report. LPA Vanegas was greeted and granted entry to the facility by Direct Support Staff (DSP) Julie Hilahan. LPA Vanegas explained the purposes of the inspection, and began a tour of the facility, and observed two clients present and were lounging in their respective bedrooms. Additional clients were at day program at this time.

LPA Vanegas left a copy of this report at facility, left a copy of amended report at facility, and conducted an exit interview with DSP Julie Hilahan.
