Facility Evaluation Report

Licensing Program Analyst (LPA) William Vanegas conducted an announced visit in order to complete a final walk through regarding an update made to the facility sketch. Upon arrival LPA Vanegas was greeted and granted entry to the facility by Direct Support Staff (DSP) Julie Hilahan. LPA Vanegas explained the purpose of the visit and began a tour of the facility and observed the following.

Clients were observed to be lounging in common areas, LPA observed no immediate risks to the health and safety of clients in care. LPA Vanegas observed the room that was updated from a staff room to a client room to be in good repair with no debris or obstructions in the room.

LPA was advised that DSP's will begin to move clients personal belongings into the new room as walk through has been complete, and is approved to have client reside in said room.

An exit interview was conducted with DSP Julie Hilahan, and a copy of this report was provided to the facility.
